Treatment Recommendation - Schedule Lab Testing 

STEP 2 - Laboratory Testing Completed at Revive Wellness

 Cost - $271.16 - Hormone Panel (fasting)

(CBC, CMP, Thyroid Panel, Ferritin, Folate, B12, Lipids, HGA1C, Insulin, Vitamin D

Testosterone free/total, Sex Hormone Binding Globulin, Estradiol -E2, Progesterone, Prolactin, FSH/LH

Cortisol, DHEA)

*Lab fee renews annually while undergoing hormone replacement therapy

 STEP 3- Follow Up Visit with Practitioner- Review Lab Results and  Discuss Treatment Options

 

STEP 4- Payment for Hormone Treatment 

Hormone Treatment with Prescription for topical/oral treatment Submitted to Pharmacy of Choice-

 Monthly Fee Covers Follow Up Visits with Nurse Practitioner, 1-Monthly B-12 Injection, Prescription/Order Submitted for Hormone Treatment

 

Cost for Testosterone Replacement Injections Submitted to Pharmacy by Provider and Mailed to Patient

-  $167.71 per Month Treatment Monitoring Fee 

Monthly Fee Covers  Follow Up Visits with Nurse Practitioner, laboratory monitoring of hormone laboratory levels during hormone treatment, Monthly B-12 Injection, Order Submitted to Compounding Pharmacy (Empower/Belmar) and mailed to client, injection supplies.

 

*Our office does not complete insurance prior authorization for hormone replacement treatment

Option A- Pay Monthly Auto-Draft by placing card on file.

*Revive Wellness does not require a contract, but you must notify the office within 7 days of scheduled auto-draft to discontinue payment

 

Option B- Pay Hormone Treatment Monitoring in 3-Month Intervals

*Not all clients are suitable candidates for Hormone Therapy. Participation in a consult 

 does not guarantee hormone therapy will be prescribed. Labs must support need, and 

 patient may not have contraindications to treatment.

* Participation in the HR Treatment plan does not guarantee  hormone therapy will be prescribed. Labs along with history and physical exam

   must support need for treatment. HRT will not be prescribed when need is not supported by laboratory values and patient symptoms. Mammogram, Pap Smear and Prostate Exam, PSA should be current to participate in Hormone Replacement
