$249 - Advanced Panel  ( Hormone Replacement & Weight Management Labs)

*When you participate  in hormone replacement this fee includes your 6-week, 3-month, 6-month lab panel. 

Cost for Hormone Replacement- $99 Monthly Practitioner Monitoring Fee Required on ALL Hormone Treatment  Clients

*Fee Includes -  Office Visits, Monthly B-12 Injection, InBody Scan, Routine Laboratory Testing, Prescription/Order for Hormone Replacement

*You may submit prescriptions to your pharmacy of choice

*Our office does not complete insurance prior authorization for hormone replacement treatment

STEP 3- Treatment Plan-  2 Payment Options Available 

Option A- Pay for 90 day treatment every 90 days during your follow up visit

Option B- Pay Monthly- Requires credit card to be placed on file for auto-draft payment

First payment due when you start treatment. Remaining payments will be completed per auto-draft and require a card be placed on file.

You may opt to have payment deducted on the 15th or 30th of each month

*Revive Wellness does not require a contract, but you must notify the office within 7 days of scheduled auto-draft to discontinue payment

Mammogram, Pap Smear and Prostate Exam, PSA should be current to participate in Hormone Replacement

*Not all clients are suitable candidates for Hormone Therapy. Participation in a consult 

 does not guarantee hormone therapy will be prescribed. Labs must support need, and 

 patient may not have contraindications to treatment.

*We do accept FSA, HSA 

*Hormone Pellets and Pellet Procedure will be an additional cost, and is not part of the HRT Package.

* Participation in the HR membership plan does not guarantee  hormone therapy will be prescribed. Labs along with history and physical exam

   must support need for treatment. HRT will not be prescribed when need is not supported by laboratory values and patient symptoms.
